The inspection request form asks for facility name, street address, city, ZIP code, facility type, machine count, sticker expiration date, contact name, email address, phone number, timeframe, and any notes you choose to add.

A request cannot be sent unless you agree to the Inspection Terms and Conditions.

If the form is not yet connected to a scheduler, what you type stays in your browser and is not sent. When the form is connected, submissions are sent so we can review the request.

We use a submission to review the request, schedule an inspection if we can take it, perform the inspection, collect payment, and file required forms with the State.

We do not sell personal information.

Payment is through Bluevine (cards, ACH, checks, and similar methods). This website does not ask for card numbers. When a Bluevine link is published, Bluevine processes payment details under its own terms.

If we connect a form host or a spam check such as Cloudflare Turnstile, that provider processes only what is needed to deliver the form and reduce unwanted submissions.

When address suggestions are available, the facility name and street text you type are sent to Google to look up the facility until that month's limit is reached. If you open the facility map, your location is sent to Google only to show nearby Indiana facilities and is not saved with the request.

This public site does not include a client directory or a filing tracker.

Inspection and billing records are kept as needed to complete the visit, file with the State, and meet ordinary business record needs.

Please do not include patient names, diagnoses, or other medical information in the request form. We inspect machines for compliance. We do not provide medical care.
