How it works

Request an inspection
Use the Request Inspection form so we have the details we need. Existing clients can reply to an automatic reminder. We schedule when our availability allows.

Prepare the office
Get the office inspection ready before we arrive. What to have ready
- 03
We complete the inspection
We visit on the scheduled day and inspect your X-ray machines.
- 04
You pay
Once the inspection is done, pay on the day of service or within 30 days.
- 05
We file with the State
We file the paperwork with the State only after payment, within 60 days of the inspection.
- 06
Stickers
Pay the same day and stickers are left with you. Pay later and they are mailed.

What to have ready
Rooms and machines
Have every X-ray room accessible and the machines working.
Facility registration
Post a current facility registration certificate from the State in the office.
Notice of Compliance
Post a current Notice of Compliance in the office and at the control panel.
Licenses posted
Post technologist certificates and other required licenses where they are easy to see.
Radiation safety procedures
Have written radiation safety procedures and rules on file, such as in a radiation binder. If you do not have them, use this template. Facility Radiation Safety Procedures and Rules (PDF)
Gonadal shielding
Have gonadal shielding available for patients and staff.
Records
If the facility was built after 1985, have model and serial numbers for certified components, the plan review, and records of surveys, calibration, maintenance, and modification. If it was built or modified after 1993, also have the safety survey on file. That survey is not part of our inspection.
Personnel monitoring
Have the monitoring company name, the types of devices used, and the number of people monitored for whole-body exposure, extremity exposure, and under age 18.
Staff counts
Have counts of licensed practitioners, state-licensed X-ray operators, other people who take X-rays, and students in approved programs.

Sticker Expiration: March 1, 2027
Final Deadline: April 30, 202760 days
You have 60 days after the sticker date, and we have 60 days after the inspection to file. Those are separate, so we cannot inspect and file on the last day. Schedule before the sticker expires. We do not recommend waiting for this deadline.
